Presenter Biography
Lelly Oboh is a Consultant Pharmacist (Care of older people) at Guy’s & St Thomas’ NHS Trust and the NHS Specialist Pharmacy Service. She is currently on secondment to lead the implementation of the National Overprescribing Review report recommendations across South East London Integrated Care System.
Lelly has over 24 years’ experience working with primary care and community multidisciplinary teams in general practice, care homes, district nursing teams, intermediate care teams and virtual wards to optimise medicines use in older people (http://www.rpharms.com/models-of-care/models-of-care-in-action.asp). She has championed the development of innovative patient-centred care models that actively engage with older people (their carers) and multidisciplinary teams during medication reviews and improve care co-ordination at transition points.
Lelly has provided expertise through membership of many strategic groups as well as NICE guideline development groups, and is Chair of the Primary Care Pharmacist Association (PCPA) Clinical Specialists Group. She is widely published, has presented at many national conferences, and has a keen interest in patient-centred approaches to care that consider the complex relationship between frailty, multimorbidity and polypharmacy as well as deprescribing, non-adherence and medicines support in social care settings.
